The following information and documents are required:

Fill in application form No. 13.
Full particulars of the applicant (Name and address).
Full particulars of the agent/representative from an applicant residing or having office outside the country (if applicable).
Power of attorney (Form No. 25), if applicable.
Three (3) reproductions of the mark/service mark.
List of goods or services according to the Nice Classification.
Certified copy of priority documents (if applicable).
Duly signed application form (applicant or agent).

The following information and documents are required:

Fill in application form No. 01.
Title of the invention.
Full particulars of the applicant/inventor (Name and address).
Full particulars of the agent/representative from an applicant residing or having office outside the country (if applicable).
Power of attorney (Form No. 25), if applicable.
Detailed specification (including abstract, claims, and drawings).
Certified copy of the priority document (if applicable).
Duly signed application form (applicant or agent).
Payment receipt for application and advertisement.
Maintenance fee for each year up to 20 years of patent protection.

Fill in application form No. 08.
Full particulars of the applicant/creator (Name and address).
Full particulars of the agent/representative from an applicant residing or having office outside the country (if applicable).
Power of attorney (Form No. 25), if applicable.
Product information.
Specimens or drawings showing the industrial design.
Certified copy of the priority application (if applicable).
Duly signed application form (applicant or agent).
Payment receipt for application and advertisement.

The following information and documents are required:

Fill in application form No. 18.
Full particulars of the applicant (Name and address).
Full particulars of the agent/representative from an applicant residing or having office outside the country (if applicable).
Power of attorney (Form No. 25), if applicable.
Geographical indication.
Geographical area.
Quality, reputation or other characteristics of the goods.
Duly signed application form (applicant or agent).
Payment receipt for application and advertisement.
